Template for Curriculum Vitae

Name and contact information

Education:  include all college information here.  Don’t need to include High School info.  Please include all degrees even if from the same institution.  Include month and year of graduation.

Professional Licensures and Certifications:  Include all current nursing licenses.  Included Certifications such as ACLS (not CPR), CNE, CCRN, etc.

Professional Experience:

·         Please start with most recent experience and work backwards.  Include dates of each experience such as Assistant Professor Nursing, North Idaho College August 2020 – Present…

·         If you had multiple roles in the same institution please include each role as a separate line item.

·         Include all your nursing roles back to the beginning of your career

Professional Activities

·         Include all committees and professional organizations, boards you have participated in.  You can determine how far back in your nursing career you want to go back.  This should be activities outside of college and nursing program activities

College and School of Nursing Activities:

·         Include all college and nursing program activities such as tenure committees, program evaluation, curriculum committee, Meet and Confer committee, etc.  include dates of service.  You should include all activities for your entire college career even if it includes activities at another school

Continuing Education:

·         List all continuing education you have completed in the last 5 years

Presentations and Publications:

Include all presentations and publications that you have made outside of your normal teaching responsibility such as Title of the presentation KH 2022

 

Prior Approval and Documentation for Professional Development Units Form

  1. To receive Professional Development Units (PDUs) for salary scale advancement, the activity must have advance approval.
  2. Faculty complete the Prior Approval and Documentation for Professional Development Units form and submit to the Associate Dean of Health Professions Director of Nursing. The forms need to be approved at least two weeks prior to the planned activity.
  3. If approved by the Associate Dean of Health Professions/Director of Nursing, the form is forwarded to the Vice-President for Instruction.
  4. Upon approval by the Vice-President for Instruction, the instructor will receive a copy of the signed form.
  5. After completion of the activity, documentation is provided to the HPN Dean’s Assistant who forwards it to the office of the Vice-President for Instruction.
  6. The HPN Dean’s Assistant maintains PDU records of faculty in the Division of Health Professions and Nursing.
