3 Filling Out the Application

The first step will be filling out the application. Keep in mind many of the state’s applications look the same with only a few minor changes. Make sure you are filling out the correct application.

This is the application we will use for applying for our LOS and when we have met all the requirements and are applying for our SOS/AOS.

Section I: Application Type

You will be applying for an Occupational Specialist.

Section II: Personal Information

In this section, fill in your personal information. The EDUID number is a state assigned number. If you do not have one or do not know you have one, leave it blank. When you get your LOS certificate, your EDUID number will be in the lower left corner. I encourage you to put that number on everything you send to the state, as it is a unique identifier.

Section III: Occupational Area and Endorsements

In this section, you will list the occupational area and endorsement you are applying for. Page 2 of the application has the list of occupational areas and endorsements. For example, if you are going to be a diesel instructor, your occupational area would be Trades and Industry, and your endorsement area would be Diesel Engine Technology.

Section IV: Education

In this section, list your education, starting with the highest degree earned. You will need official transcripts. However, we will discuss that in another chapter. If you need more room, include a list with the application.

Section V: Licensing History

In this section, you will need to answer the yes/no questions. If you answer yes to any of the questions, you will need to provide a written explanation.

Section VI: Legal History

In this section, you will review the legal statement.

Section VII: Attestations and Signature

In this section, you will attest by initialing the statements. Once you have initialed the statements, the final step for the application is to sign/date the application.
